It just provides insurance with the extra knife but you'd be surprised, it really goes a long way. I tested it a week ago, it only works for the specified shoulder charge melees and not grapple melee. If in the perk description it is written that it works with grapple melee then it works, if it is not written then it does not work. It counts as melee for Heavy handed and Assassin's Cowl.
